If you wish to change an existing appointment, please complete the form below. A member of our team will then contact you via email or text message within three business days with your new appointment. Please note that whilst we always endeavour to accommodate preferred dates, our surgeons are in high demand and so this is not always possible. However, if there are dates when you are simply not available, then please set those out below and we will make sure we give you an appointment to suit.
